Erectile dysfunction (ED) or impotence is sexual dysfunction characterized by the inability to develop or maintain an erection of the penis during sexual activity. A penile erection is the hydraulic effect of blood entering and being retained in sponge-like bodies within the penis. The process is most often initiated as a result of sexual arousal, when signals are transmitted from the brain to nerves in the penis. The most important organic causes are cardiovascular disease and diabetes, neurological problems (for example, trauma from prostatectomy surgery), hormonal insufficiencies (hypogonadism) and drug side effects. Psychological impotence is where erection or penetration fails due to thoughts or feelings (psychological reasons) rather than physical impossibility; this is somewhat less frequent but can often be helped. Notably in psychological impotence, there is a strong response to placebo treatment. Erectile dysfunction can have severe psychological consequences as it can be tied to relationship difficulties and masculine self-image. I'm excited to speak about the role of robotics in Prostate Artery Embolization. So a couple of things I'm going to cover here. First, is just review the literature for PAE and describe a little bit about who's the candidate for the procedure, then focus on what the challenges are with the procedure, because think that's the main obstacle that we face with PAE. And then, the transition of the Magellan Robot into our current practice with PAE, and then what those actual advantages are over the traditional method. And I'll show some case examples. So in terms of who is the candidate, there's really a few ways to break this down. The first is looking at what does the data support in terms of which patients to treat, who are poor traditionally urologic candidates, and let's talk about who's not really a candidate at all for PAE. So the data, when we look at a review article from 2014, this is published in CBIR, and it reviewed all the studies done up until mid last year. And as you'll see on the next slide, it looked at 562 studies through a PUBMED database study. They ended up narrowing it down to nine articles, in which they reviewed 706 patients that were actually included in the analysis. There was some possible overlap of data from European studies, and there were no randomized control trials at that point published or studied longer than two years. All patients had moderate-to-severe symptoms in terms of their prostate enlargement, or BPH, and the mean age of the patient in these studies is, as you would expect, in the mid to late 60s to early 70s. When they took all the data together and they pulled it all together, they found out that although these patients started in the moderate-to-severe category, as you can see here in the dark blue line, this is an IPSS score, which is the severity of symptoms a man faces from their prostate enlargement.

5 Silent Signs You May Have Clogged Arteries! Natural Remedies The arteries of our bodies are vital to our health. Without a fully-functioning system of arterial vessels, the blood in our body cannot be effectively delivered to our organs. In identifying with any of these signs, we have the opportunity to seek medical care before serious health conditions arise. HERE ARE 5 SIGNS YOU MAY HAVE CLOGGED ARTERIES: 1. LOWER BACK PAIN The arteries of our lower back are among the first areas to accumulate plaque. As such, lower back pain is a common first symptom of artery blockage. Lower back pain can occur when reduced blood flow to the back weakens the disks that cushion the vertebrae – leading to painful herniated disks and pinched nerves. An arterial defect in the back area is a quite common condition. Consider that roughly 10% of the population will demonstrate advanced blockages in their lower back by age 20. With this knowledge, it is beneficial to our children that they receive a physical at least once a year. In a study done by the European Journal of Vascular and Endovascular Surgery, individuals that suffer with chronic back pain are at an increased risk of developing clogged lumbar arteries. 2. CALF PAIN Arteriosclerosis – the thickening and hardening of the arterial walls – can also block the arteries that deliver blood to the legs. When this happens, it is common to experience pain in one or both calves, a condition known as claudication. Those that smoke are in a higher risk class than those who do not. Regardless, if this symptom is detected, a doctor’s evaluation should be conducted ASAP. Simple blood tests will be conducted that examine the pulses of the legs, in addition to a couple of other basic tests. 3. DISCOMFORT OR PAIN OF THE LEGS AND FEET Peripheral artery disease (P A D) is a buildup of cholesterol and plaque that can occur in the extremities. PAD often results in discomfort in the feet and legs, potentially limiting the ability to walk. In worse cases, P A D is advanced enough to result in the loss of a limb. Once again, simple tests will identify arterial conditions such as P A D. During a routine medical exam, a doctor will check the pulse of the feet. In addition, the doctor may also conduct a screening that determines the amount of blood flow to the ankle. People that have frequent pain or tiredness in the legs should be checked for PAD. Past or current smokers have an increased risk of the disorder, as do those with a family history of P A D or other cardiovascular problems. 3. EAR CREASES One of the more odd signs of clogged arteries is ear creases. Now, a momentary pause while about half of our readers stand in front of a mirror…welcome back! Seriously, medical reports have mentioned ear creases as a silent sign of coronary heart disease (CHD). More specifically, an ear crease that runs diagonally from the ear canal to the lower edge of the earlobe. This is a rather contentious sign of clogged arteries, although it may be worthwhile to schedule a routine checkup. Of course, ear creases can also be another physical sign of aging. 5. ERECTILE DYSFUNCTION Once again, the effects of clogged arteries directly relate to decreased blood flow. Our (ahem) private members are no different in this regard. Maintaining an erection requires that blood have an unobstructed path to this area. According to WebMD, Erectile Dysfunction at least indicates that some of the blood vessels on that path are not in ideal health. As a precaution, it’s a good idea to schedule a doctor visit to discuss these symptoms. In conclusion, having any of these signs is not a surefire indication of clogged arteries. Pain in the legs An extremely common PAD symptom is claudication, a type of leg pain or discomfort. Because the arteries are clogged, they can't deliver enough blood to the legs to support exertion. The pain can be in any part of the leg, from the calf to the thigh or buttock, and it may be in one or both legs. It's also reproducible: The pain happens when walking a certain distance (like two blocks), it's relieved by rest, and then occurs again when walking the same distance. 2. Nighttime cramps While sleeping, people with PAD may get cramps or spasms, typically in the heel, forefoot, or toes, says Darren Schneider, MD, director of the Center for Vascular and Endovascular Surgery at New York–Presbyterian Hospital/Weill Cornell Medical Center. The pain can often be relieved by dangling the foot off the bed or sitting in a chair, which allows gravity to assist blood flow to the feet, Dr. Schneider says. 3. In some people with PAD, the feet or toes are pale or bluish when they're sitting, due to a lack of circulation. 5. Cold feet Feet or legs that feel cold, or are cool to the touch, may be an indicator that you have PAD. But it's not really the best indicator, says Dr. Schneider. That's because this is a common problem, and it can happen to anyone as he ages—even someone without PAD.
